I assume all responsibility for obtaining medical or other professional help regarding any health problems that could be identified with this program.
All of my questions regarding the program have been satisfactorily explained.
Memberships are not subject to refunds. However, memberships may be put on hold due to illness with prior documentation. Agreements paid monthly may be cancelled only if member moves more than 25 miles away. Proof of new address and cancellation fee of $75 US is required if the term of the original agreement is not met.
Proper Gym Attire - Blue jeans, khakis and non athletic shoes are NOT acceptable.
No STREET Clothes allowed.
No chalk permitted.
No loitering on the gym floor.
Re-rack your weights and dumbbells.
Do not DROP or THROW your weights.
Please leave TV volume down - wear a headset.
Be courteous of fellow gym members - avoid excessive screaming, yelling and swearing.
Children under the age of 12 may NOT be on gym floor without a FF Certified PT.
Children ages 12-15 may ONLY be on the gym floor under the direct supervision of a parent.
Children may NOT be brought to childcare if sick: too sick for school, too sick for childcare.
Space in fitness classes is on a first come first serve basis.
Shower before entering pool and/or jacuzzi.
Do not use pool area as a walk through.
Wipe down equipment after use. Towels/gym wipes are provided for your convenience.
Childcare operating hours:
Monday – Friday
8:00am – 1:00pm & 3:00 – 8:00pm
Saturday – 8:00am – 12:00pm
2 Hour limit PER DAY for childcare! 1 Hour limit for infants under 12 months.
Childcare is Closed Sundays
The ages for childcare attendance are 6 months to 12 years. Childcare is for members’ children ONLY.
There is a limit of (3) children per member. Additional children are $5/child.
Childcare ratio must be 12:1 and maximum capacity is 24 children: 2 staff.
Infants should be fed and have a clean diaper when dropped off. Our staff will not change diapers or feed.
Any child displaying symptoms of illness or contagious symptoms will be asked to leave. These symptoms include fever, discolored runny nose, rashes, excessive cough, etc.
Toddlers should be taken to the restroom prior to drop off. If your child is not fully potty trained, have him or her wear a pull up.
Only the adult signing in the child will be allowed to pick up the child. This will be strictly enforced to ensure the safety of all children in our care.
We ask that you do not enter the Kids’ Forum room unless absolutely necessary. This can be disruptive and upsetting to the children.
We will correct all children in a loving way, but if they continue any disruptive behavior and/or excessive crying (10 minutes), you will be notified to pick your child up.
No running, jumping or horseplay is allowed. Continued violations will result in your child being suspended from Kids’ Forum.
NO SNACKS/FOOD are allowed in Kids’ Forum. Spill proof cups ONLY.
We will NOT ACCEPT children 30 minutes prior to closing.
Please label your child’s diaper bag, backpack, & spill proof drink containers.
Should your child have any allergies – please alert the childcare staff. While we will do our best to protect your child, please understand that you do assume the risk of your child being inadvertently exposed to allergens.
Shoes must be worn at all times. Parents, do NOT allow your children to RUN as they enter/exit the club. In addition to the risk of a slip or fall, we have many elderly patrons who could be seriously injured in a collision with your little one.
For the safety of all the children, we ask that small objects not be brought into the room. (i.e.: coins) We will remove these items and return them to the parent. We also ask that children not bring toys from home. This tends to cause conflict and is disruptive.
Electronic games may be brought, but staff will not be responsible for any loss or breakage.
Parents are not to leave Fitness Forum with child/children in the childcare.
For your child’s safety, we ask that they not be allowed on or near any of the exercise equipment while entering or exiting the childcare.
If you and/or your child cannot communicate with our staff or understand our rules, you may not utilize the Kids’ Forum amenity.
Fitness Forum has a qualified staff. While we will do our best to love and care for your children, there are inherent risks involved when children play in a group setting.
These rules are designed to ensure that your child/children have a safe and clean environment while you exercise. Remember that this is NOT a one-on-one babysitting service; it is a play center. Please be courteous to our staff. They do their best to keep your children safe and happy.
